The Illinois Organ/Tissue Donor Registry is a confidential computerized database managed by the office of Illinois Secretary of State Jesse White that documents your wishes to become an organ/tissue donor. Your confidential registration ensures that your wishes to donate will be honored at the time of your death; additional witnesses or consent from others is no longer required.

To join the registry, simply complete all fields in the form below. Your information will be entered in the registry, and you will receive a confirmation letter in a few weeks.
